A Genuinely Remarkable Drug
Ivermectin became a political argument in 2020, and the argument buried an extraordinary story.
In the 1970s, Satoshi Ōmura isolated Streptomyces avermitilis from a soil sample near a golf course in Japan. William Campbell, working at Merck, found that compounds from that organism were exceptionally active against parasites. The result was ivermectin.
Ōmura and Campbell shared the 2015 Nobel Prize in Physiology or Medicine for it. The citation was for a drug that has "radically lowered the incidence of river blindness and lymphatic filariasis."
Merck then did something almost unheard of: in 1987 it committed to donate ivermectin free of charge, for as long as needed, to everyone who needed it for onchocerciasis. The Mectizan Donation Program has since delivered billions of treatments. River blindness has been eliminated in several countries in the Americas.
That is the drug. It is one of the most consequential medicines of the twentieth century, and it belongs to parasitology.
How It Works
Ivermectin binds glutamate-gated chloride channels — ion channels found in the nerve and muscle cells of invertebrates. Binding opens the channels, chloride floods in, the cell hyperpolarises, and the parasite is paralysed and dies.
The reason this is safe in humans is precise: mammals do not have glutamate-gated chloride channels. We have related GABA-gated channels, but they sit behind the blood-brain barrier, and ivermectin is actively pumped back out of the brain by P-glycoprotein.
That specificity is the whole basis of the drug's safety margin, and it also explains its limits. A drug that targets an invertebrate ion channel is superbly designed for worms and has no equivalent target in a virus or a human tumour cell.
What It Is Actually Approved For
In the United States, FDA-approved human uses:
Oral ivermectin (Stromectol and generics)
- Strongyloidiasis of the intestinal tract — the drug of choice
- Onchocerciasis (river blindness)
Topical ivermectin
- Rosacea — Soolantra, 1% cream, approved for inflammatory lesions of rosacea. Covered in Ivermectin for Skin
- Head lice — Sklice, 0.5% lotion
Beyond the formal labels, ivermectin is widely used and recommended by CDC and WHO for scabies — including as first-line oral treatment for crusted scabies and for institutional outbreaks — and in mass drug administration for lymphatic filariasis.

That is a substantial list of genuine, evidence-backed human indications, and most of the argument about this drug has caused people to lose sight of it.
Safety, Honestly
At approved doses for approved indications, ivermectin has an excellent safety record across billions of doses.
Common side effects are usually mild: dizziness, nausea, diarrhoea, itching, rash.
The genuine danger is Loa loa. In people heavily infected with the African eye worm, killing large numbers of microfilariae rapidly can cause severe encephalopathy. This is why mass treatment programmes screen for it in co-endemic regions.
Interactions. Ivermectin is a P-glycoprotein substrate. Drugs affecting that transporter can raise brain concentrations. It also interacts with warfarin.
Overdose is real. Reported effects include vomiting, hypotension, confusion, ataxia, seizures and coma.
The Veterinary Formulation Problem
This is worth stating bluntly because it caused genuine harm.
Veterinary ivermectin — horse paste, cattle pour-on, injectable — is formulated and concentrated for animals weighing several hundred kilograms. A single horse paste tube is dosed for a 1,200-pound animal.
Poison control centres recorded a sharp rise in calls related to veterinary ivermectin ingestion, with cases of confusion, hallucinations, seizures, hypotension and hospital admission. Pour-on formulations contain solvents intended to cross animal skin and are not made to be swallowed.
If ivermectin is the right treatment, it is available as a human prescription with a known dose. The veterinary product is a different formulation at a different concentration, and the difference is not academic.
What It Is Not
It is not an antiviral. The proposed antiviral mechanism came from a laboratory study showing reduced viral replication in cell culture at concentrations far above anything achievable in a person at safe doses. Large randomised trials — including ACTIV-6 and TOGETHER — did not find clinically meaningful benefit for COVID-19. FDA, EMA and WHO do not recommend it for that use outside trials.
It is not a cancer treatment. There is preclinical research, and it is discussed honestly in Ivermectin and Cancer, but there is no human clinical evidence of efficacy.
Saying so is not a dismissal of the drug. Ivermectin does not need borrowed credentials. It won a Nobel Prize for what it actually does.
If You Think You Need It
Get the parasite identified. Ivermectin is specific — it works on some organisms and not others, and it will not treat a tapeworm or giardia. Do You Actually Have a Parasite? covers testing.
Then get a prescription. Dose is weight-based, some indications need repeat dosing, and strongyloidiasis in particular needs follow-up because the organism can persist for decades.
